Chapter Bi-Annual Report

All CBM Chapters should complete the attached Chapter Bi-Annual Report (formerly known as Chapter Information Form) to provide updated information regarding chapter membership (status and changes), annual reports, planned youth activities and organizational initiatives sponsored throughout the calendar year. Please forward the completed report to the CBM National Office either via e-mail at info @cbmnational.org or by U.S. Postal Delivery ATTN: Director of Operations.
